New Patient Information
Your Initial Office Visit
We are always accepting new patients. Our office sees patients as young as 14 (with up-to-date immunization records). Before your initial visit, we ask that you please print out and fully complete our new patient paperwork:
Along with your paperwork, please bring your licence and most up-to-date insurance card(s).
If you are going to bring any of your own medical records, we ask that you please only bring the most recent preventative testing (labs, specialists’ reports/testing, colonoscopy, mammogram, dexa scan).
Insurance Information
We accept most major commercial and HMO insurance plans as well as Medicare, Champus/Tricare and many others.
We do not accept Medicaid, AmeriGroup, Worker’s Compensation or Motor Vehicle Accident cases.
We suggest contacting your insurance company to see whether or not we are 100% a participating provider in your network.
